1. I hereby agree that every application is subject to the eligibility criteria set by Koperasi Dana Ikhlas Berhad. If my application does not meet the prescribed criteria, the Cooperative will refund the share capital of RM1,000.00 (Ringgit Malaysia: One Thousand Only) within ten (10) working days from the date of notification of application rejection.

2. I have read and understood the Cooperative By-Laws and agree that, in accordance with sub-regulation 14(1) of the Regulations, I declare: 

• (a)I shall be bound by these By-Laws and activity rules currently in force and any lawful amendments made thereto during my membership period;
• (b) I am not an undischarged bankrupt;
• (c) I am not subject to any active criminal conviction that is recordable; and
• (d) I have not been expelled from the membership of any cooperative within the past one (1) year.

3. At any time, should I cease to be a member of this cooperative, I hereby bequeath to the Accountant General’s Department of Malaysia all contributions and/or any financial entitlements remaining in this cooperative, should efforts by the Board of Directors to contact me or my next-of-kin for a period of one year fail.
